PCM Academy exists because specialty practices across the country are delivering PCM-qualifying care every single day — and almost none of them are billing for it. We built the playbook that closes that gap.
Amy Post brings more than 20 years of expertise in healthcare leadership, population health, and specialty practice operations. Before founding PCM Academy, she spent 22+ years at Abbott and AbbVie — two of the largest healthcare companies in the world — where she partnered with specialty practices, multi-specialty groups, health systems and specialty pharmacies to implement value-based care initiatives.
Over her career, Amy has sat on both sides of the value-based care table. She has worked inside pharma on HCP-facing programs, inside provider organizations on care-management rollouts, and alongside CMS-aligned programs covering Medicare Care Management, billing and compliance, EHR workflow integration, staff training, and patient engagement strategies.
She saw the same thing everywhere: specialty practices delivering the exact care coordination Medicare designed PCM to reimburse — and leaving the money on the table because no one had packaged the implementation.

Certified in Value-Based Care (Harvard T.H. Chan School of Public Health), Population Health (Thomas Jefferson University, School of Population Health) and Lean Healthcare (Portland State University), Amy launched PCM Academy to address one critical gap: specialty practices were delivering PCM-qualifying care but lacked the tools, workflows, and confidence to bill for it.
Today, Amy empowers practices, health systems, and their partners nationwide — building sustainable PCM programs that benefit patients and providers in equal measure. PCM Academy's methodology has been adopted by specialty practices, multi-specialty groups, health systems, and specialty pharmacies.
When Amy isn't collaborating with practice managers, she's leading cohorts, refining the curriculum to align with the latest CMS updates, or thoughtfully crafting the next iteration of the 12-Step Accelerator.
